Our transitional care is the first 60 days after being discharged from a facility, the health care provider coordinates a patient’s health care needs with other health care providers. The health care provider manages a patient’s chronic conditions as the patient transitions from being in a facility to being at home.
PRIMARY/CHRONIC CARE
Our primary care or chronic care is a provider who cares for a patient’s full medical condition and coordinates care with other health care providers.
